Submit your practice and provider information, and preferences to get started with Allergenis. Complete the form online here, or download the account setup form below.

step 2: place your order

Ordering is quick and easy with drop-downs on our provider portal and requisition and ICD-10 guides to help you along the way. Once you have completed your account set up form, we will email you your portal access information. You can also download the requisition from below to get started prior to receiving account information.

step 3: provide a specimen or request mobile phlebotomy

Follow our step by step instructions for how to draw, process the specimen, complete the requisition, package, and ship. If you choose mobile phlebotomy, please “mobile phlebotomy request” into the Notes section when you approve the order on the portal.

step 4: review results

Color coded results allow for quick interpretation. Also the report includes an easy-to-read graphical representation of the epitope reactivity levels. For additional insight into understanding the report, view the test results report guide.
